For contractors in the City of Buffalo, five-part contracts are available for order either through this website or in person at Vilardo Printing. A minimum order of ten sets is required, priced at $2.50 each, totaling $25. With tax, the total payment will be $27. If you want to order more than ten sets, please indicate on the form how many sets you wish to order. Each additional copy is $2.50 plus tax. 

 

Please note that payment is accepted only by check or cash at the time of pickup, and there is a minimum turnaround time of 24 hours. 

 

Please fill out the form below. Make sure all of your information is correct. The name of your business MUST match the name on your business certificate. This includes all commas and periods. The City of Buffalo will reject your contract forms if they do not match exactly.
